EP-473 Laparoscopy versus Laparotomy for Abdominal Trauma: a Case of Iatrogenic Haemoperitoneum in a Haemodynamically Unstable patient

Abstract Introduction Laparoscopy is the gold-standard approach to elective abdominal surgery. Nevertheless, its application trauma, including treatment of haemoperitoneum, has been historically challenged by links missed injuries. The European Association for Endoscopic Surgery does not clearly recommend therapeutic laparoscopy in despite recognised potential benefits, light scarcity and poor standardisation available evidence. Case description A 68-year old male, presented with worsening shortness breath ascites. He was type-2 respiratory renal failure on a background decompensated heart failure. admitted ITU circulatory ventilatory support underwent ascitic drainage, removed day 2. On 3, patient became peritonitic, exhibited haemoglobin drop went into class 2 haemorrhagic shock. CT-abdomen showed moderate An emergency undertaken, which revealed active bleeding from left lower wall. Haemostasis achieved ligation electrocautery deep inferior epigastric perforators. Post-operatively, sustained clinical improvement. 4 he stepped down ward 10 discharged without further complications. Discussion Therapeutic efficacious safe selected cases intra-abdominal as emerging literature. haemodynamic stability always pre-requisite, patient's preoperative comorbidities can be key deciding favour laparoscopy.
